2019 Volunteer Screening Process

Good Morning Coaches and Managers,

As we prepare for our upcoming season, I have attached, below, the police reference check waiver form for all volunteers to fill out and submit to Toronto Police Services. Please carefully read and distribute the following information to relevant parties as this process is intended to be completed before the indoor training sessions in February.

Waiver:

  1. Please print the attached waiver form on legal size paper only (New waiver form – old one has been discontinued)
  2. Please state on the application form :

Agency: SCARBOROUGH MINOR FOOTBALL ASSOCIATION 

Agency Contact Person:  STEPHANIE BOIRE 

  1.  All applicants must be residents of Toronto
  2. Applications must be signed and either dropped off or mailed to:
                    Toronto Police Service
                    Police Reference Check Program
                    40 College Street
                    Toronto, ON
                    M5G 2J3
  1. A fee of $20.00 for volunteers  must be enclosed (Money order, certified personal cheque to Toronto Police Service)
  2. If you choose to drop off the waiver in person - there is a drop-off box located in the lobby of Police Headquarters at 40 College Street. Please understand that there is an approximate wait time of 10-15 business days to fully process the application. 
  3. Your processed application will be directly mailed to your home address and must be submitted to Scarborough Minor Football Association for us to have on file.
If there are any questions or concerns regarding this process - please email me directly as I will try my best to help. The process is different this year as we are now a registered agency with TPS and bulk submissions are no longer processed.
